Name of Society A40  Name of society in which membership is held.
Post-Held A10 Currently held executive position in the society.
Membership  A10 Type of membership held, e.g. Honorary, Junior, Full, etc.
Join Date DD-MON-YYYY Date on which current type of membership was obtained.
