Probation Assessment Form {P00013}

This program allows the end user to complete the sections of Probation Assessment Forms assigned to him/her.

• A Routing Request {GROU-2} with an associated Probation Assessment Form for a personnel member is automatically created by the Personnel Biographical {PBOP-1} - program when the Probation Date- and a Probation Period  (other than 0) is entered for the person.
• The Responsible Person Type and - Number of each Sequence of the Routing Request indicates the personnel member to which the section of the Probation Assessment Form associated with the Routing Sequence is assigned.
  

• Each section of the Probation Assessment Form is identified by the Routing Sequence and consist of one or more Question Group Sets with one or more Questions each as set-up inMaintain Probation Assessment Questionnaires { PAOP1-1}.
• Display will be sorted by Question Sequence within Question Group Sequence and will prompt for responses using the Answer Type set-up as done inMaintain Probation Assessment Questionnaires { PAOP1-1}.
• Questions that require a mandatory response will be flagged with a "*".
• For Question Groups that repeat, a button flagged with "Add a Row" is available to allow the end user to enter multiple responses
  

• This program will also display completed (previous) sequences of the Probation Assessment Form, although no changes will be allowed.
